Purpose of the trial: To test effectiveness of theophylline in contrast-induced nephropathy (CIN) prevention in well hydrated population. Methods: A randomized, double-blind, placebo-controlled trial. 56 patients undergoing coronary angiography (CAG) and/or percutaneous coronary intervention (PCI) were enrolled, 31 to theophylline group (200 mg IV before procedure) and 25 to placebo group. This case shows history of therapy of ileofemoral thrombosis in a so-far healthy patient. Medical therapy was complicated with heparin-induced thrombocytopenia type II (HIT II). HIT II is a complication of therapy with unfractionated heparin (UFH) or with low molecular weight heparin (LMWH). HIT II can be easily diagnosed, when considered. Not diagnosed HIT II can lead into major complications, because of serious acquired state of hyper coagulation that requires a specific access and therapy. The purpose of this work is to summarize basal theoretical findings with an emphasis on possible mistakes as it is documented in the case report.

FINESSE study didn´t confirm expected benefit of PCI facilitation in acute myocardial infarction by half dose reteplase with abciximab or abciximab alone before transportation to cathetrisation centre. The results could have been affected adversely by enrollement of patients, who presented from 3 to 6 hours after the beginning of symptoms, suboptimal antiplatelet therapy or immediately performed PCI in cases when TIMI flow 3 was reestablished after pharmacologic intervention.
