Acute pulmonary embolism is one of frequent life-threatening cardiovascular diseases. Its severity varies from low risk cases with good prognosis to severe shock states with high mortality. The most exact diagnostics and risk stratification is basic requirement for successful therapy. The introduction of highly-sensitive method for the assessment of cardiac troponins represents a progress in diagnostics, therapy and risk stratification in acute coronary syndromes. It provides earlier diagnosis and increase of the sensitivity in the diagnostic process of acute myocardial infarction. Further increase of the sensitivity can be achieved with simultaneous assessment of other markers (copeptin, heart fatty acid binding protein) or evaluation of additional clinical parameters (electrocardiogram, GRACE or TIMI risk score). of the diagnostic catheter requiring surgical removal Percutaneous coronary angiography is commonly performed examination with a low risk of a serious complications. Occurrence of complications of transradial approach is around 1 %. In our institution, we perform about 2 500 coronarography per year since 2008, about 1100 PCI, approximately 90 % is performed by transradial approach, and most of the patients examined in the outpatient mode. We present a case report of severe complication of transradial coronary angiography, where it was necessary surgical intervention, but without effect on patient΄s prognosis.

V této kazuistice autoři prezentují případ 65leté ženy s anamnézou dušnosti a kolapsů. Echokardiogafickým vyšetřením byl zjištěn velký tumor v levé síní, velmi suspektní jako myxom. Magnetická rezonance potvrdila morfologii a tvar útvaru, jehož tkáňová charakteristika byla při vyšetření diagnostikována jako trombus. Po antikoagulační léčbě došlo k vymizení útvaru. Kazuistikou chceme dokumentovat důležitost včasného echokardiografického vyšetření a následně magnetickou rezonancí, která může určit i tkáňovou charakteristiku útvaru.

Paravalvular leak (PVL) is one of the most serious complications after valve replacement. If it is haemodynamicaly significant the therapy of choice is conventional open heart surgery. Percutaneous closure of the leak is the alternative method for smaller leaks. In following case report we describe hybrid surgical-interventional transapical closure of paravalvular leak of mitral valve. We report a case of a 63 years old man after repeated sternotomy, in whom paravalvular leaks after mitral valve replacement occured.
