Introduction: Catheter alcohol septal ablation (ASA) is an effective treatment of patients with symptomatic hypertrophic cardiomyopathy associated with left ventricle outflow tract obstruction (HOCM). Currently, there is a lack of data describing the long-term effectivity of ASA after ultra-low dose of alcohol (≤ 1 ml). Aim: To assess the effect of ASA based on the amount of applied alcohol in long-term follow-up. Methods: The prospective cohort of 84 consecutive patients with the average age of 59 ± 4.2 years (62 % females) was divided into two groups. Group A, 31 patients treated with ≤ 1 ml of alcohol and Group B, 53 patients...

Cardiocenter Hospital Podlesi TAVI registry Introduction: Transcatheter aortic valve implantation (TAVI) together with the surgical replacement belong to the life-saving therapeutic procedures. Aim: The main purpose of this registry was to evaluate the short-term clinical outcome of patients after TAVI in Cardiocenter Hospital Podlesi Trinec. Methods: From July 2009 to December 2013 a total number of 236 patients underwent TAVI for native aortic valve stenosis or regurgitation (98.3 %) and degenerative aortic bioprosthesis (1.7 %). Patients‘ data were prospectively uploaded to the Czech TAVI Registry, which is maintained by the Institute...

The assessment of right ventricular systolic function is extremely important in all patients undergoing cardiothoracic surgery in both perioperative period and in long term follow up. The gold standard for routine assessment of right ventricle (RV) remains transthoracic echocardiography. Contraction of the RV is predominantly determined by longitudinal myocardial fibres of the ventricular wall, particularly by those present in the inflow tract. Therefore parameters reflecting longitudinal shortening of the RV free wall, namely tricuspid annular plane systolic excursion (TAPSE) and peak systolic velocity of tricuspid annulus (St), are recommended...

Clopidogrel treatment starting before transportation for primary PCI is better then in-hospital administration according to observational studies. Prasugrel is most potent contemporary drug for STEMI treatment, but data for prehospital administration are insufficient. Prehospital administration of ticagrelor is as effective as in-hospital treatment.

Surgical treatment for ischemic heart disease went through a long historical development. So called Vineberg´s procedure was routinely performed in early fifties of the last century as attempt for heart muscle blood supply improvement. We are presenting a patient at whom first surgery was performed in 1974. Later on PCI for left anterior descending artery (LAD) stenosis failed. Left internal mammary artery (LIMA) was natively intact. MIDCABG approach was indicated because of adhesions from previous surgery. Surgery with extracorporeal support cannulas inserted for safety purposes was uneventful. Patient was released into the rehabilitation...

Primary angioplasty (PCI) in old saphenous vein grafts (SVG) is often accompanied with distal embolization leading to worse angiographic outcome and unfavourable prognosis. Thrombus aspiration and distal protection device, in connection with low pressure stent implantation, were succesfully used to prevent progression of the acute heart failure during PPCI in the old SVG.

The review article summarizes clinical pharmacology, pharmacokinetics, efficacy, safety of and clinical experience with apixaban. Apixaban is a novel oral anticoagulant with a rapid onset of action and a mechanism of direct, selective inhibition of coagulation factor Xa. The results of the ADVANCE clinical trials have shown that apixaban is comparable to or more effective than low-molecular-weight heparin enoxaparin in preventing venous thromboembolic events in patients who underwent total knee or hip replacement, with a similar or lower risk of bleeding. Other large clinical trials confirmed that apixaban was significantly more effective than...
