Primary angioplasty (PCI) in old saphenous vein grafts (SVG) is often accompanied with distal embolization leading to worse angiographic

outcome and unfavourable prognosis. Thrombus aspiration and distal protection device, in connection with low pressure stent

implantation, were succesfully used to prevent progression of the acute heart failure during PPCI in the old SVG.

Keywords: old saphenous vein graft, acute myocardial infarction, distal protection
