Interv Akut Kardiol. 2014;13(2):59-61

Purpose: The aim of the study was to evaluate the acute vascular changes after application of RF energy within the renal denervation using intravascular ultrasound. Methods: In 4 patients with resistant hypertension referred for renal sympathetic denervation selective angiography and intravascular ultrasound examination (IVUS) was performed in total of 9 renal arteries before and after procedure. Results: Angiographically the caliber of renal arteries in the entire length of the intervened segment decreased in total of 0.37 ± 0.14 mm (from 5.45 ± 1.14 mm to 5.08 ± 1.12 mm, p = 0.004). According to IVUS the vessel area did not...

A survey of patients treated for infective endocarditis at cardiac-surgery department

Interv Akut Kardiol. 2014;13(2):63-66

of University Hospital Ostrava in 1998–2012 Purpose: Infective endocarditis is a disease with a relatively low incidence and high mortality. In our study, we present the results of a range of patients operated at cardiac-surgery department of the University Hospital Ostrava for acute infective endocarditis in 1998–2012. Methods: This is a retrospective series of patients admitted to our department in the years 1998–2006 (group A) and in the years 2007–2012 (group B) with a diagnosis of acute infective endocarditis by DUKE criteria. We followed and compared in each file location of affected valve, indication for surgery,...

Interv Akut Kardiol. 2014;13(2):68-72

Objective: To explore the value of routine mean platelet volume (MPV) determination in patients with the acute coronary syndrome. Methods: We compared MPV in 333 persons divided into two groups. In the first group, there were patients hospitalized with an acute coronary syndrome (myocardial infarction or unstable angina pectoris). Second group was represented by patients with stable angina pectoris and healthy blood donors. Analyzers Cell Dyn Saphire and XL 5000 were used to determine MPV. Results: We proved statistically significant difference in the MPV value between patient with ACS and the group of healthy donors and patient with stable...

Novel oral anticoagulants and acute myocardial infarction

Ivo Varvařovský

Interv Akut Kardiol. 2014;13(2):73-75

Novel oral anticoagulants (NOAC) are considered better then warfarin for prevention of ischemic stroke and systemic embolization in nonvalvular atrial fibrillation. Direct thrombin inhibitors and factor Xa inhibitors have a different influence on the risk of myocardial infarction. Considering the risk of myocardial infarction should be part of the decision-making process for long-term NOAC therapy.

Dual antiplatelet therapy in patients after surgical revascularization of myocardium

Andrej Myjavec, Miroslav Brtko

Interv Akut Kardiol. 2014;13(2):77-85

Aortocoronary bypass graft surgery with the use of arterial and/or venous grafts is well-established method of cardiac revascularization. While the long-term patency of arterial grafts is outstanding, venous grafts are burdened with quite a high rate of occlusions, potentially posing a risk of myocardial infarction, necessity of revascularization and death to the patient. Acetylosalysic acid is indicated in all patients with ischemic heart disease (in the absence of contraindications). Its early administration is important in order to prevent venous graft occlusion as well as to affect morbidity and mortality in the patients after the surgical...

Arrhythmogenic right ventricular cardiomyopathy

Luděk Haman

Interv Akut Kardiol. 2014;13(2):87-91

Arrhythmogenic right ventricular cardiomyopathy is a genetic form of cardiomyopathy usually transmitted with an autosomal dominant pattern. It primary affects the right ventricle, but may involve also left ventricle and lead to biventricular heart failure, ventricular tachyarrhythmia’s and sudden cardiac death. Pathologically is characterized by myocardial atrophy and fibro-fatty replacement. Diagnosis is difficult due to broad spectrum of phenotypic variations, therefore diagnostic criteria combined history, electrocardiography, echocardiography and magnetic resonance imaging have been developed. Basic diagnostic evaluation is early...

Clinical experience with novel oral antithrombotic drugs and some questions so far unanswered

Jan Vojáček

Interv Akut Kardiol. 2014;13(2):92-98

The recent introduction in the market of several novel oral anticoagulant (dabigatran, rivaroxaban, and apixaban) and antiplatelet drugs (ticagrelor and prasugrel) has been encouraged by positive results of pilot studies and has resulted in a massive explosion of publications, in part supported by companies and, in part, based on rapidly gained extensive experience with novel antithrombotic drugs. The overall clinical benefit of anticoagulant therapy is apparent virtually in all patients with nonvalvular atrial fibrillation, except for those with a CHA2DS2-VASc score of 0. If the CHA2DS2-VASc score is used, the proportion of patients with atrial...

Acute anterior myocardial infarction caused by trombotic occlusion of LAD in patient with acute methanol intoxication

Karin Poloková, Ota Hlinomaz, Roman Panovský, Věra Feitová, Vladimír Zemánek, Luděk Pluháček

Interv Akut Kardiol. 2014;13(2):100-102

This case report describes the 39-years old patient who underwent acute methanol intoxication complicated by acute anterior myocardial infarction caused by trombotic occlusion of left anterior descending (LAD) artery. There were several cases of methanol intoxication in Czech Republic in autumn 2012 because of distribution of poor quality spirits. Part of those patients died due to severe metabolic acidosis and brain oedema. A typical complication is blindness. In literature, hemodynamic complications are also described – depression of left ventricular function, hypotension, ECG abnormalities and arrhythmias, dyspnoea, lung oedema, cyanosis....
