of hypertrophic obstructive cardiomyopathy with alcohol septal ablation Introduction: Alcohol septal ablation (ASA) is together with septal myectomy respected method of treatment in patients with hypertrophic obstructive cardiomyopathy. There are only scarce experiences using transradial approach in ASA. Purpose of the study: To evaluate acute and mid-term results of ASA performed transradialy. Methods: ASA was performed from the right radial artery, pacemaker lead was inserted using subclavian approach, the left ventricle pressure was measured by pigtail catheter inserted transfemoraly. Actual changes in ultrasonography and catetherization...

Thromboembolism, coagulation and atherothrombosis

Tomáš Kvasnička

Interv Akut Kardiol. 2012;11(1):15-17

The epidemiological and clinical relations between venous thromboembolism and increased incidence of cardiovascular diseases are intensively studied at present. The classic concept of atherosclerosis indicate a pivotal role of inflammation in the development and progression of this disease. Atherothrombosis is a disease specific for „a modern“ population and is the most common disease, which significantly increases mortality and impaires prognosis of the patients. This is a generalized process affecting large and medium size arteries in a human body. Experimental data indicate that platelets and the coagulation system are important...

Biomarkers of myocardial overload

Jan Krupička, Tomáš Janota

Interv Akut Kardiol. 2012;11(1):18-21

The worldwide incidence of heart failure is steadily increasing over the past several decades, partly due to the population aging and improved survival of patients with cardiovascular diseases. Therefore raises the importance of biochemical substances which would uncover ongoing cardiac overload, enable the treatment monitoring and make care of the patients with heart failure more effective. According to results of many clinical trials, this task is fulfilled at most by natriuretic peptides which gradually become a part of standard clinical practise. In patients with acute as well as chronic heart failure we frequently detect higher plasma...

Indications for cardiac resynchronization therapy: recent large trials and analyses of specific subgroups of patients

Kamil Sedláček, Josef Kautzner

Interv Akut Kardiol. 2012;11(1):22-27

Cardiac resynchronization therapy has become an integral part of the therapy spectrum in patients with advanced heart failure. Guidelines for cardiac resynchronization therapy issued by the Czech Society of Cardiology in 2008 were based on the preceding version of European guidelines for the treatment of heart failure and cardiac resynchronization therapy. European guidelines have been updated in 2010 after results of major randomized trials in patients with mild heart failure became available. In this article we compare individual versions of the guidelines and discuss indications for cardiac resynchronization therapy in specific subgroups,...

Beta blockers in treating chronic heart failure in patients with impaired renal function

Filip Málek

Interv Akut Kardiol. 2012;11(1):29-32

The decline of kidney function is an important negative prognostic parameter in chronic heart failure. Use of betablockers is safe and effective also for the subgroup of the subjects with decreased renal function. The evidence of the beta blockade efficacy in the patients with depressed kidney function is indirect. Cardioprotective effect of betablockers is obviously independent on the degree of renal damage. The information of the betablockers effect on the outcome of the heart failure patients with renal insufficiency gets along with the results of the prospective cohort studies as well as the results of mortality trials subanalyses.
