Is radial approach appropriate for beginning interventional cardiologists ?

Lukáš Jaworski, David Horák, Rostislav Polášek

Interv Akut Kardiol. 2010;9(6):286-288

Advantages of radial access (AR) compared to femoral access (AF) were demonstrated in many clinical trials. Experience with training of new interventional cardiologists using primarily radial access are missing. This article compares both approaches in the hands of beginning interventional cardiologists performing diagnostic cardiac catheterizations. Retrospective analysis of datas confirmed benefit of radial access. Fluoroscopic times were comparable (AR vs AF 5.56 ± 0.24 min/examination vs 5.75 ± 0.27 min/examination; P = 0.699), total times of examinations were significantly shorter (AR vs AF 21.18 ± 0.59 min/examination vs...

The effect of sequential therapy on the neurotic disposition of patients who have suffered from coronary heart disease

Dana Brabcová, Jana Miňhová, Jiří Kohout, Marta Zárybnická, Blanka Šestáková

Interv Akut Kardiol. 2010;9(6):289-292

This research explored neurotic disposition in patients who had suffered from cardiological disease and were in recovery. The study examined the personal history and socio-demographic characteristics of two groups of men; one from the Faculty Hospital Pilsen (n = 130) and the other from the Höhenklinik Bischofsgrün in Germany (n = 84). The establishments differ significantly in their approach to sequential therapy which can in turn affect the psychological state of the patient. We used the 48-question Middlesex Hospital Questionnaire (MHQ) to determine neurotic disposition as a whole as well as disposition to specific categories of illness...

Spontaneus coronary artery dissection

David Richter, Jiří Ostřanský, Martin Sluka, Roman Štípal, Miloš Táborský

Interv Akut Kardiol. 2010;9(6):294-297

Spontaneous coronary artery dissection (SCAD) is very rare and associated with high mortality, in clinical practice most often presented by acute coronary syndrome, involving frequently younger women. Treatment should be conservative, interventional or cardiosurgical. Authors describe 4 cases of SCAD in cath lab during last 5 years.

Electromagnetic interference in patients with pacemakers and implantable cardioverter defibrillators

Vlastimil Doupal, Miloš Táborský

Interv Akut Kardiol. 2010;9(6):298-301

Electromagnetic interference is still actual problem in patients with active cardiologic implantable devices. The risk of the most common sources of electromagnetic interference and recommendations for interference with pacemakers and implantable cardioverterdefibrillators prevention are reviewed.

Is there any link between myocardial reinfarction after a major orthopedic surgery

Tomáš Roubíček, Vladimír Hraboš

Interv Akut Kardiol. 2010;9(6):316-318

and a lack of effectiveness of antiplatelet therapy ? Extensive orthopedic procedures, such as total hip replacement, represent an important thrombotic state of the organism as a response to tissue injury. We present a 63 years old patient who suffered an acute myocardial infarction as a first symptom of coronary heart disease early after orthopedic surgery. Myocardial infarction was treated by urgent percutaneous coronary intervention of the right coronary artery. The patient was taking dual antiplatelet therapy (aspirin and clopidogrel). The patient suffered myocardial re-infarction due to stent thrombosis on day 9. Optical aggregometry...

Transaortic balloon counterpulsation

Jan Hlavička, Karel Jirásek, Tomáš Vaněk, Zbyněk Straka

Interv Akut Kardiol. 2010;9(6):320-322

A case report of an insertion without vascular prothesis and with delayed sternal closure Transaortic balloon counterpulsation is an alternative method used in patients with critical lower limbs ischemia or abdominal aortal aneurysm, both preventing classical implantation via femoral artery. Case report describes the application of the method without a commonly used graft, with temporarily open sternum and a beneficial subsequent effect.

Genes or chance ?

Tomáš Fiala

Interv Akut Kardiol. 2010;9(6):323-325

We report the case of two sisters catheterized for stable angina pectoris CCS II. Clinical manifestation occurred after reaching almost the same age and both women had similar findings at coronary arteries. With regard to their older ages in the time of clinical manifestation – 74 years – the result in this case was probably due to interaction of similar genetic and environmental influences.

Regression of left ventricular hypertrophy after successful endovascular treatment of aortic coarctation in an adult

Jan Šochman, Jiří Novotný, Jan H.Peregrin

Interv Akut Kardiol. 2010;9(6):326-328

Coarctation of the aorta is a cause of secondary arterial hypertension. It is usually associated with the left ventricular wall hypertrophy of varying degree. A 58-year-old man with coarctation of the aorta, a bicuspid aortic valve, moderate aortic regurgitation, long standing arterial hypertension and left ventricular wall hypertrophy was treated by stent graft placement. The treatment was successful, controlled hypertension decreased afterload beyond the incompetent valve and lead to unexpected as well as very rare regression of the left ventricular wall hypertrophy.

Antithrombotic prophylaxis - treatment according to guidelines or according to the patient's needs ?

Jan Bultas

Interv Akut Kardiol. 2010;9(6):302-308

Implementing the knowledge of pharmacogenetics into the treatment of cardiovascular diseases has moved from the phase of theoretical assumptions to that of practical use, which is particularly true for antithrombotic treatment. Polymorphism of transformation enzymes, the transport systems governing the fate of drugs in the organism, as well as of receptors, enzymes or signalling molecules affecting drug action usually underlies the interindividual differences in the effects of drugs. In antithrombotic treatment, polymorphism of transport proteins (P-glycoprotein) in resorption or polymorphism of CYP2C19 isoenzyme involved in clopidogrel bioactivation,...

Dronedarone - new antiarrhythmic drug for treatment of atrial fibrillation

Kateřina Lefflerová

Interv Akut Kardiol. 2010;9(6):310-315

Dronedarone is new antiarrhythmic agent developed for the treatment of atrial fibrillation. It is a noniodinated benzofuran derivate of amiodarone with electrophysiological properties similar to amiodarone but without pulmonary and thyreoid toxicity. Dronedarone has been proven to maintain sinus rhythm and to control ventricular rate during episodes of atrial fibrillation. Patients with severe heart failure should not receive dronedarone. It si a first antiarrhythmic drug, which reduced cardiovascular hospitalisation and mortality.
