STRESS SPECT IMAGING AFTER REPEATED REVASCULARIZATION

A 71-year-old female was referred for stress SPECT study because of exertional chest pain 6 months after CABG. She had a left internal mammary graft to the left anterior descending artery (LAD), and vein graft to the right coronary artery (RCA). Stress 201Tl SPECT images: an extensive reversible defect was seen in the LAD artery territory. Repeated coronary angiography revealed the left internal mammary graft stenosis in distal anastomatic lesion and percutaneous coronary intervention (PCI) was performed. Patient was restudied 4 months after PCI because of atypical chest pain and dyspnea. Repeated stress 201Tl SPECT study is normal, demonstrating significant improvement in myocardial perfusion after PCI.

Jednasedmdesátiletá žena byla odeslána na zátěžové SPECT vyšetření pro námahovou bolest na hrudi šest měsíců po CABG. Pacientka měla našit arteriální štěp z levé vnitřní mamární tepny na ramus interventricularis anterior (RIA) a venózní štěp na pravou koronární tepnu (ACD). Zátěžové 201Tl SPECT vyšetření ukázalo rozsáhlý reverzibilní defekt v povodí RIA. Rekoronarografie prokázala těsné zúžení v místě anastomózy arteriálního štěpu a byla provedena perkutánní koronární intervence (PCI). Pacientka byla znovu vyšetřena čtyři měsíce po PCI pro atypickou bolest na hrudi a dušnost. Opakované zátěžové 201Tl SPECT vyšetření je normální, svědčí pro významné zlepšení perfuze myokardu po PCI.
