Primary versus secondary transport in patients with acute ST-segment elevation myocardial infarction, times and mortality rates

The aim of our study was to show to what extent direct transport of patients with ST-segment elevation myocardial infarction (STEMI) from the field to a catheterization laboratory affects shortening of the time-to-reperfusion with primary PCI and what effect it has on their mortality. Although the management of patients with STEMI in prehospital care has dramatically changed in the recent years, still a non-negligible proportion of patients are transported to regional hospitals and only afterwards to a catheterization laboratory.

Authors present rewiev of current findings and data concerning the diagnostics and percutaneous treatment of significant left main stem coronary artery disease. They bring out anatomical and histological aspects of this specific localization of coronary atherosclerosis, assessment of its morphological and functional significance, comparison with surgical revascularization, present perspective on options and techniques of percutaneous intervention, as well as current guidelines for its indication.

Peri-operative care of patients with antikoagulantion therapy

The management of anticoagulation therapy in peri-operative setting is a common problem, because of required balancing between thromboembolic risk and hemorrhagic risk arising from surgery. Evidence from randomized studies regarding the optimal peri-operative approach is lacking. This review summarizes the available evidence for peri-operative management in patients on anticoagulation therapy. At the same time presents a practical approach for optimal peri-operative strategy in this group of patients.

Symptomatic giant pericardial cyst

Pericardial cysts are rare mediastinal abnormalities with an incidence rate of 1 per 100,000. They are mostly asymptomatic, discovered incidentally during routine imaging examination. The management of pericardial cysts depends on their size and symptoms, where asymptomatic ones are usually observed, while symptomatic ones are indicated for a surgical resection or percutaneous aspiration. We report the case of patient with giant pericardial cyst presenting with chest pain and precollapse, successfully extirpated using video- assisted thoracoscopic surgery.

Implantation of cardioverter-defibrillator in gravidity

The number of women with congenital heart diseases and cardiomyopathies rises in our population with increasing amount of these women reaching adulthood and planning pregnancies. Gravidity brings plenty of physiological changes, which might have arrhytmogenic influence on myocardium with genesis of both supraventricular and ventricular arrhythmias. Serious reasons force us to consider implantation of cardioverter-defibrillator (ICD) during pregnancy. This is a very rare performance that can be undertaken with fluoroscopic or echocardiographic control. In the literature there are only few cases of ICD implantation during gravidity to be found....
