Perfusion strategy in patients with functioning internal mammary artery grafts indicated for reoperative aortic surgery

Vladimír Lonský, Martin Kaláb, Petr Šantavý, Petr Caletka, Vítězslav Pavlíček, Martin Volt

Interv Akut Kardiol. 2009;8(4):178-181

Introduction: Patients undergoing aortic surgery after CABG in the presence of patent IMA represent a potentially high-risk group. The presence of patent pedicled IMA grafts poses a significant risk and requires specific considerations for myocardial protection (washout of cardioplegia in the IMA teritory) and prevention of IMA grafts injury. Various approaches to myocardial protection in the presence of patent IMA have been proposed. Patients: We performed 3 aortic REDO operations with this technique last year. Mean age of our patiets was 79 years (71–83). Mean interval between the first operation and reoperation was 8 years (5–11),...

Surgical treatment of aortic valve disease in the elderly

MUDr. Eva Koudelková CSc

Interv Akut Kardiol. 2009;8(4):182-185

The number of elderly patients undergoing aortic valve surgery is increasing with the increase in life expectancy. The symptoms in septaand octogenarians are often underestimated due to limited physical activity and they are reffered for surgery too late when advanced calcifications of aortic valve as well as left ventricular systolic dysfunction in elderly men and diastolic dysfunction of the left ventricle in elderly women are present. More than half of these patients require concomitant coronary artery bypass grafting and secondary mitral insufficiency and pulmonary hypertension are also frequent. The prevalence of preoperative co-morbidities...

Cardiovascular rehabilitation of patients after surgical revascularization of myocardium

Ivan Karel, Hana Skalická

Interv Akut Kardiol. 2009;8(4):186-190

The authors submit the instructions for procedures performed in the course of the cardiovascular rehabilitation following a myocardial revascularisation. The listed procedures are in full accordance with the recommendations of the Czech Cardiological Society (on which both of the authors had participated) and are justified by the conclusions from similar works from the worldwide written presentations as well as by their own results. They prove the high effectiveness of cardiorehabilitation that supports and develops the effect of a cardiosurgical act, improves the subsequent quality of life, decreases morbidity as well as mortality, influences...

Tailor-made antiplatelet treatment in patients after percutaneous coronary intervention - a dream of the future or high time to take the first step ?

Róbert Ševčík, Jan Vojáček, Hana Ševčíková, Jaroslav Dušek, Josef Bis, Josef Šťásek

Interv Akut Kardiol. 2009;8(4):192-195

Dual antiplatelet therapy is standard for the patients after acute coronary syndrome for many years. The combination of acetylsalicylic acid (ASA) and clopidogrel is reducing acute and subacute stent thrombosis under 1 %. There is enough of evidence that the response to most of the drugs, including antiplatelet drugs varies considerably among individuals. Residual platelet reactivity despite of antiplatelet therapy is related to increased risk of thrombotic complications after percutaneous coronary intervention (PCI). Although many assays are sensitive to examine platelet inhibition, there is only weak association among platelet function test results....

The chronic aortic dissection - an atypical angina pectoris as an atypical symptom

Karel Lukeš, Martin Urban, Piotr Branny

Interv Akut Kardiol. 2009;8(4):196-199

This report describes a case of a young man with the dissection of ascending aorta, that was presented by atypical angina pectoris. The atypical, position dependent angina pectoris to the diagnosis of coronary artery disease. Also cardiovascular diagnosis was considered. When the cause of the disease was determined, the patient was treated by supracoronary replacement of the ascending aorta.

Solitary metastasis of renal cell carcinoma in the right ventricular outflow tract

Lukáš Vlk, Marek Šetina, František Mošna, Tomáš Tláskal, David Zemánek, Theodor Adla, Ivan Kawaciuk

Interv Akut Kardiol. 2009;8(4):200-203

Solitary metastasis of renal cell carcinoma invading interventricular septum occurs very rarely and in the Czech literature has not been presented up today. We describe such a patient in our case report, who underwent nephrectomy and lymphadenectomy, extirpation of the tumor of the interventricular septum, reconstruction of the interventricular septum and tricuspid valve as well, and single mammarocoronary bypass to the left anterior descending artery.

Rosuvastatin

MUDr. Michal Vrablík Ph.D

Interv Akut Kardiol. 2009;8(4):204-207

Rosuvastatin has been introduced into the Czech market only recently. It possesses beneficial pharmacological properties and the greatest lipid-lowering potency of all available statins. Clinical trials with rosuvastatin using atherosclerosis paging (ASTEROID, METEOR) have shown its ability of slowing atherosclerosis progression and even inducing regression of atherosclerotic vascular lesions. In the CORONA and AURORA studies, adding rosuvastatin to medication of heart and kidney failure patients was not associated with any benefits. On the contrary, in the JUPITER trial, which included patients with low LDL-cholesterol levels at baseline rosuvastatin...
