Angiographic and clinical assessment of the first generation of drug eluting stents in the consecutive patients with high risk of restenosis

Purpose of the trial: To evaluate mid-term results in the unselected population after first generation drug eluting stent implantation. Methods: We conducted a prospective angiographic and clinical analysis of 89 consecutive patients with implanted drug eluting stent in the time period from February 2004 till November 2005. We evaluated angiographic in-segment restenosis obtained by QCA at 12 months. Clinical endpoints were death, infarction and/or target lesion revascularisation (TLR) at 12 months, with clinical restenosis analysis. Aims: To evaluate one-year experience with CT examination of the heart using dual-source CT (Somatom Definition, Siemens, SRN) at the department without previous experience with CT examination of the heart. Methods: All patients were examined on dual-source CT Somatom Definition. Two hundred eleven patients were examined from February 2nd 2007 to January 31st 2008. Authors present a review of current literature on basic life support without mouth-to-mouth ventilation (Hands-Only CPR) which definitely changed approach of lay rescuers to victims suffering from unexpected cardiac arrest. Historical rationale of integration of mouth-to-mouth breathing into the Basic Life Support algorithm, its adverse effects and recent role in relation to pathophysiology of the most frequent type of cardiac arrest in adults are discussed. Radial artery perforation

Iatrogenic radial artery perforation is a rare complication of transradial catheterization. It is associated with minimal risk of patient injury in case of gentle instrument manipulation, early diagnosis and treatment. The authors describe four cases of this complication.

Unusual cause of angina pectoris

A 71-year old patient underwent surgical revascularization LIMA-LAD for the tight proximal LAD stenosis 6 years ago. For the last several months, he had been suffering from angina pectoris, rating from moderate to unstable, and finally including attacks of angina at rest. Coronary angiography revealed normal LIMA-LAD; however, a severe stenosis of the subclavian artery proximal to the LIMA take-off was found as the cause of the angina pain. Subclavian artery stenosis was managed by successful stent implantation and the patient has remained free of any symptoms for one month after the procedure.

A case of an old lady with symptomatic aortic stenosis and ischemic heart disease is presented. She was treated by Transcatheter Aortic Valve Implantation (TAVI) after previous PCI of left anterior descending artery with drug eluting stent implantation. The technique of TAVI is described.

Aortic valve Edwards SAPIEN™ is one of the two systems for percutaneous replacement of a degenerative stenosed aortic valve in the market. We describe a transfemoral implantation of an Edwards SAPIEN™ valve and also we discuss current criteria for indication of a percutaneous aortic valve implantation. We publish the first experience with implantation of the two Edwards SAPIEN™ aortic valves in the 1st Department of the Internal Medicine University Hospital Hradec Kralove, Czech republic.
