The coronary cutting balloon angioplasty - experience with treatment of restenosis in stent, ostial and bifurcation stenosis

Background: Cutting balloon coronary angioplasty has been recently introduced into clinical practice. This procedure leads to controlled dissection of vessel wall by 3 or 4 very small blades – atherotomes that are attached to balloon catheter. Long-term ECG monitoring in patients with unexplained syncope - first experiences with implantable loop recorder

Conventional tests have known limitations in the diagnosis of syncope. Implantable loop recorder (ILR) is capable to monitor continously ECG over a period of several months with an option to record signals during a period of symptoms after post hoc telemetric activation. The aim of the study was to evaluate the significance of ILR in patients (pts) with recurrent syncope of unknown origin. ILR was implanted in 48 pts (29 female), mean age 51±17 years, (range 16-78 years) with unexplained syncope. Venous and arterial thrombophilia

Thrombophilia is a state of an increased tendency to the occurrence of thrombosis in veins and arteries. In both cases, it represents a multifactorial disease with different pathogenesis. In venous thrombosis, ceased blood flow and hypercoagulation are the main culprits, on the contrary, platelet activation and endothelial dysfunction in atherosclerosis are responsible in arterial thrombosis. A survey is given on known mutations or candidate phenotypes which are associated with thrombophilia and their practical point of view. Further clinical studies are warranted before we can definitively tell what tests are suitable for genetic diagnosis of thrombophilia.

In the past, intraoperative balloon coronary angioplasty was considered a useful method, which can improve outcome of coronary artery by-pass grafting. This approach has been abandoned due to technical difficulties. Authors refer a case of 74 years old woman with tight ostial stenosis of the left main coronary artery due to aortic valve replacement with bioprothesis 2 years ago. Her condition was complicated by significant paravalvular leak and left ventrical dysfunction.
