Pseudoaneurysm of the left ventricle is relatively rare complication of myocardial infarction. The risk of the rupture is much higher than
in the true left ventricular aneurysm. The early diagnosis and differentiation of these two clinical comlications is crucial for the best
decision to treat surgically. This text summarises knowledge of the pathogenesis, focusing on the possibility to achieve the true diagnosis
early and noninvasively, including cardiac computed tomography and cardiac magnetic resonance. We present the clinical case of
79-year-old woman with pseudoaneurysm of the left ventricle as a late complication of an old myocardial infarction.
