Authors present two cases of patients who came to the hospital for the first out-patient examination with severe heart failure, dilatated
left ventricle and a very low ejection fraction. After the start of the therapy, the next diagnostic step was a coronary angiography. The
results of coronary the possibility of coronary revascularization (1). Echocardiography plays the central role in the next controls (2). In
case of resting heart rate is more than 70 per minute, ivabradine administration is indicated to established pharmacotherapy of the
heart failure (3, 4). The improvement of the ejection fraction helps the patients to improve quality of life and to work a part – time job (5).
