Postavení direktního stentingu v léčbě akutního koronárního syndromu

Implantace stentu se stala běžnou součástí perkutánních koronárních intervencí (PCI), která se stále častěji provádí u pacientů s akutním koronárním syndromem (AKS). Konvenční stenting (CS) je postupně nahrazován přímou implantací stentu bez nutnosti předchozí predilatace - direktním stentingem (DS). Ten má četné výhody i některé limitace. Přibývá literárních údajů o přednostech této metody nejen u pacientů s chronickou ICHS, ale i s AKS včetně infarktu myokardu. Článek pojednává také o prvních zkušenostech s DS při implantaci drug eluting stentů (DES).

POSITION OF DIRECT STENTING IN THE TREATMENT OF ACUTE CORONARY SYNDROMES

Stent implantation became a routine part of percutaneous coronary intervention (PCI) which is performed more often in patients with acute coronary syndrome (ACS). Conventional stenting (CS) is partially replaced by direct stenting without predilatation (DS) which has many advantages and some limitations. There is an increasing number of literature about advantages of this method not only in stable patients but also in ACS including acute myocardial infarction. This article also describes first experience with DS using drug eluting stents (DES).
