První hypotézu o paradoxní embolizaci otevřeným foramen ovale vyslovil v roce 1877 Conheim při pitvě mladé ženy, která zemřela na cévní mozkovou příhodu. S rozvojem jícnové echokardiografie se podařilo řadě autorů zachytit přímo průchod velkého hadovitého venózního trombu otevřeným foramen ovale. Častější než tyto velké tromby jsou však malé, několikamilimetrové tromby, které embolizují do plicního řečiště a zde podléhají spontánní lýze bez klinických následků. Pokud se však tyto malé tromby dostanou při zvýšení tlaku v pravé síni otevřeným foramen ovale do systémového řečiště, např. mozkového, mohou být následky jejich embolizace daleko závažnější.
